Trauma Triage

Purpose: To provide guidance to field personnel and the BioTel staff when making destination decisions for traumatized patients. Contact BioTel as soon as possible to allow adequate preparation of the receiving trauma center.

These patients must be taken to a designated Level 1 or Level 2 trauma facility:

  • Traumatic arrest/CPR (with signs of life on EMS arrival)

  • Systolic B/P less than 90 mmHg

  • Decreased level of consciousness (LOC) with GCS of less than 10

  • Penetrating injuries to head, torso, and neck RTS of less than 10

  • Amputations (proximal to wrist or ankle)

  • Multi-system blunt trauma with unstable vital signs (BP less than 90 mmHg, RTS less than 11, GCS less than 14)

  • Amputations (with reimplantation potential)

  • Paralysis, numbness, or other signs of spinal cord injury

  • Flail chest

  • Open or suspected depressed skull fracture

  • Unstable pelvis or suspected pelvis fracture

  • Two or more long-bone fractures

  • Fall greater than 10 feet

 

Strongly consider transport to a designated Level 1 or Level 2 trauma facility for the following, in the judgment of the paramedic and/or in conjunction with BioTel:

  • High energy event

  • Auto-pedestrian impact

  • Motorcycle or bicycle involvement

  • Patient greater than 65 years of age

  • Ejection from the vehicle

  • Death of occupant in same vehicle

  • Rollover mechanism

  • Bent steering wheel

  • Significant aggravated assault
